2. Middle Name

3. Last Name

4. Date Of Birth

5. Gender

6. Disability status.

7. Phone Number

8. Must be having either the National ID or Maisha Card

10. ID Number

11. District of Birth

12. County

13. Sub County

14. Ward / Location

15. Village / Informal Settlement

16. Residence

17. Level of education: Primary, Secondary, Undergraduate, Vocational, Graduate or Masters.

18. Skill level: Skilled (Industrial trade), Skilled (Service trade), Semi-skilled or Unskilled.

19. Preffered Job: Construction works or Malaria Youth Army.

After registration, a selection committee will be convened to select those who will participate in the program. Only successful applicants will be notified via phone (SMS) of selection into the program.
